What is Revision Rhinoplasty?

Revision rhinoplasty is also called secondary rhinoplasty, and is performed subsequent to another nose surgery or surgeries. Often, revision rhinoplasty is performed to correct abnormalities that resulted from another nose surgery, but it can also correct damage from injury or trauma to the nose.

In some cases, revision rhinoplasty is needed to fix functional problems with the nose that resulted from the initial surgery. Some patients who have rhinoplasty for cosmetic reasons end up with breathing problems because of the modifications made by their initial cosmetic surgeon, and may require sinus surgery. Other patients are unhappy with their cosmetic results, and in some cases there are both functional and aesthetic problems after the first surgery. How is Revision Rhinoplasty Performed?

Revision rhinoplasty is a much more difficult procedure than primary rhinoplasty for several reasons. The initial surgery may have removed too much bone or cartilage, which will need to be replaced with grafts in the secondary procedure. Also, the scar tissue that is left after the primary rhinoplasty will pose a challenge during the revision, as will changes in skin thickness. If breathing was impaired as a result of the initial nose surgery, the internal functional features of the nose will need to be restored. Clearly, revision rhinoplasty is best performed by a highly experienced nose surgery specialist.

Dr. Nachlas will access the internal anatomy of the nose either through incisions inside of the nostrils, called closed rhinoplasty, or he will make an incision across the underside of the tip of the nose, called open rhinoplasty. Revision procedures typically require the open approach, in which case Dr. Nachlas can sometimes re-use the original incision site to prevent additional scarring. Repairing the underlying structures of the nose in a revision rhinoplasty can take up to 4 hours, and usually requires general anesthesia.

Recovery and Results

The nose is firmly splinted after surgery to assure that it holds its new shape while healing. There will be bruising and swelling, as well as nasal stuffiness. Prescription or over the counter pain medication is usually used for a few days after surgery to manage discomfort. As the swelling subsides and you return to a more presentable appearance, you will gradually return to your normal activities. Most patients need to take at least a week off of work for this procedure.
